Course Selection Form (The link was emailed to parents on Wednesday, March 10.)
Print and sign, then scan or take a picture, and upload the forms here (http://m.unioncatholic.org/uploadforms) or request a paper copy here (http://m.unioncatholic.org/paperrequest)

All five forms must be submitted at the same time.
